Family & Community Services Offerings

Through clinical services, we empower individuals and families to attain and maintain the essential social, emotional, and adaptive skills to navigate their current environments. We prepare individuals for increased independence in education, employment, and community living settings.

Who We Serve

  • Children, Adolescents, Teens and Young Adults
  • Autism Spectrum Disorders
  • Intellectual disabilities
  • Developmental disabilities
  • Mental Health Diagnoses
  • Experience of trauma, including post-traumatic stress disorder and abuse and neglect
  • Clients with delays in the realms of adaptive, daily living, and social skills
  • Clients with delays and/or added supports needed to effectively communicate, ie: augmentative and alternative communication (AAC)
  • DCFS-Involved and Post-Adoption/Guardianship Families

Who We are Not Equipped to Serve

  • Clients who have been determined to require more than outpatient, community-based care.
  • Clients who exhibit extreme behaviors that require more than 1 staff
  • Clients who have severe medically complexities (as primary service intervention need)
  • Clients who are actively abusing substances
  • Clients with a diagnosis of Conduct Disorder and/or severe difficulties with conduct that would negatively impact service intervention and/or extreme risk of safety to self and others.
  • Clients who exhibit sexually aggressive behaviors presenting a risk of harm to self and/or others.

1. Contact Little City Community-Based Clinical Services.

2. Clinical Director will work with you to determine service eligibility and to gather referral information.

3. If eligible for services, a clinician will be assigned and will contact you to schedule an initial assessment.

4. Initial assessment will be completed, and a treatment plan developed.

5. Services will be provided to align with treatment plan.

6. Treatment plan will be reviewed every 6 months, and services adjusted as needed.

7. Discharge from services will occur when treatment goals or other discharge criteria are met.

In-person services available in Cook County, DuPage County and Will County; telehealth services available anywhere in Illinois.
